Physical Exam

Getting an annual physical exam, or well check-up, is important to your overall care. We recommend you come in once a year to screen for various conditions and prevent disease. In fact, most insurance plans encourage it, sometimes with no cost to you!

Note: If you are a Medicare patient, please see information in your free Medicare Annual Wellness Visit, which is different from physical exam.

What to expect

We will review your medical history and risk factors. Depending on your health, age, gender, health history, your exam may include:

  • Physical exams to check vital signs and health
  • Discussion of diet, exercise, tobacco use or alcohol consumption
  • Screenings for breast or cervical cancer or prostate cancer
  • Blood test or other labs, such as diabetes and cholesterol screenings
  • A referral for other screenings if appropriate, such as a mammogram or colonoscopy, bone density test, or an aortic aneurysm screening
  • You may be offered an update on your vaccines

Limitations: This is a “well” visit only

Your check-up will take the full visit time. There will not be time to discuss illnesses or chronic diseases — and it would not be covered in the cost of the exam.

You will need to schedule another appointment to discuss anything that is not part of a normal annual preventive exam. The following are not included in the annual exam:

  • chronic disease, such as hypertension or diabetes
  • acute health issues like a cold
  • prescription medicine refills

Come prepared: write it down!

Please come prepared with the information in this check list to make the most of your exam and help us better partner in your care.

  • Any notable changes in health
  • Recent labs, diagnostic test, or immunizations (bring your immunization card if available)
  • Your current prescription and over-the-counter medications with dosages
  • Names of vitamins and supplements regularly taken
  • Any updates to your medical history: surgeries, hospitalizations, new allergies, and new diagnoses from other physicians
  • Updates to your family medical history
  • For females: First day of last menstrual period
  • Names of other physicians you are seeing and what you are seeing them for
  • Questions you have about recommended screenings or lifestyle considerations to maximize health
